The objective of this section is to record any issues, any issue being a concern, a question, a point raised but not clear what the next step will be, etc. From issues the points will become Actions, Risks or Escalations to combined group activity. Some judgement needs to be exercised as to the significance of the issue, but as a rule of thumb this section would expect to have between 10 and 30 issues/risk/actions open at any one time.

When initially raised the Status is Issue. This is changed to Action if a way to deal with the issue is identified, or to Risk if identified as such. In both cases the Follow Up will contain the action to be carried out or the risk mitigation. If the issue is identified as requiring wider discussion then Status becomes Escalation
